VA 22-1995 Enrollment Form

Thank you for choosing to further your education as a Certified Heavy Equipment Operator with Heavy Construction Academy!  You’ve decided to utilize your military educational benefits to complete this 6-week training program.

Since you have used your Post 9/11 benefits in the past you will need to complete the VA 22-1995 form below to continue in the HCA enrollment process.

PLEASE NOTE: This form MUST be filled out  on a laptop or desktop computer.  You cannot complete this form on your smart phone.
